Transitional Care Program (TCU)CRMC-East, 2600 E. 18th Street,
Cheyenne, WY 82001 Transitional care/skilled nursing helps meet the needs of patients who no longer require acute hospital care, but still need nursing and inpatient treatment because of complications or unresolved medical conditions. Therapy is delivered at a lower intensity than acute rehabilitation.
CRMC provides services for people with the following conditions: Amputation Our approach to care: An individualized treatment program is developed for each patient based on an evaluation at time of admission. The treatment team may consist of a physician, rehabilitation nurses, physical therapists, occupational therapists, speech/language pathologists, a social worker, a dietician, and an activities coordinator. Program goals may include: · Increased independence
with the activities of daily living Our commitment to delivering the highest quality rehabilitation services. Anyone can initiate a referral or elect our program.
