Transitional Care Program (TCU)

Transitional care/skilled nursing helps meet the needs of patients who no longer require acute hospital care, but still need nursing and inpatient treatment because of complications or unresolved medical conditions. Therapy is delivered at a lower intensity than acute rehabilitation.

  • Anyone can initiate a referral or elect our program.
  • A screening assessment will be conducted on-site free of charge.

CRMC provides services for people with the following conditions:

Amputation
Multiple Trauma
Orthopedic Injuries
Cancer
Stroke
Cardiopulmonary Conditions
Neurological Disorders
Post-surgical Conditions

Our approach to care:

An individualized treatment program is developed for each patient based on an evaluation at time of admission.

The treatment team may consist of a physician, rehabilitation nurses, physical therapists, occupational therapists, speech/language pathologists, a social worker, a dietician, and an activities coordinator.

Program goals may include:

· Increased independence with the activities of daily living
· Increased mobility
· Disease self-management
· Home evaluation
· Patient and family education
