Going Home:We strive to make your stay as comfortable as possible, but we also know you are eager to go home. Remember, our nursing case managers and social workers are available to assist you, your family and your physician. Discharge Planning: Should you require services beyond our hospital setting, social workers and nursing case managers are available to assist you and your family (following the recommendation of your physician). This includes transfers to a nursing home, rehabilitation facility or another hospital. Also, they acquaint patients and families with available community resources such as home-care agencies, Meals-on-Wheels, homemaker services, etc. Our case managers and social workers can also help you order special home-care equipment, such as beds, wheelchairs and miscellaneous supplies. The case mangers and social workers are based on each unit. Ask the unit staff for assistance in contacting them. The mission of Home Care Services is to offer competent, compassionate care, primarily in the home, through all phases of life. Emphasis is placed on efficient, comprehensive quality care which supports the physical, emotional, social and spiritual needs of the people served. Home Care (633-7000) offers skilled care under the direction of a physician, and consists of health services provided by licensed professionals. |
